The Disability Resource Center of Panama City is dedicated to supporting individuals with disabilities through crucial life transitions. Our Transition Services are designed to facilitate a seamless move from one stage of life to another, ensuring that every individual has the support, resources, and guidance they need to thrive in their chosen path. This core service is divided into two main areas: Youth Transition and Nursing Home Transition.
Youth Transition
Our Youth Transition services are tailored to assist high school students with disabilities in navigating the path from public education to post-secondary education, employment, and other life settings. We understand the challenges and uncertainties that come with this transition, and our goal is to make the process as smooth and empowering as possible.
Services include:
- Personalized planning to align with each student’s aspirations and abilities.
- Workshops and counseling on post-secondary education options, including vocational training and college.
- Job readiness training, including resume writing, interview preparation, and job search strategies.
- Connections to community resources and services that support independent living and personal development.
Nursing Home Transition
Our Nursing Home Transition services focus on assisting individuals with disabilities who wish to avoid placement in nursing home settings or those looking to transition back into the community. We believe in the right of every individual to live in the least restrictive environment possible, and our services are designed to support this belief.
Services include:
- Comprehensive assessments to understand each individual’s needs and preferences.
- Planning and coordination of services to support living in the community, including home modifications, in-home support, and access to community resources.
- Advocacy and assistance in navigating healthcare systems, funding sources, and legal rights.
